  Parent request to participate and medical release form. Medical Information is only necessary for Overnight Trips/Retreats.
- PERMISSION SLIP AMENDMENT
  As of 2024, ONLY TRIPS OVER 100 MILES from the Diocese of Biloxi require notarization.
- ADULT MEDICAL FORM
  Medical release and information form for adult volunteers. Medical Information is only necessary for Overnight Trips/Retreats.
- DRIVER INSURANCE FORM
  Volunteer Driver Information Form concerning youth events.
VIRTUS, Safe Environment Training and Background Checks

The Diocese of Biloxi is committed to protecting the dignity, safety, and well-being of all who place their trust in the Church. ALL adult volunteers must be VIRTUS-compliant and background checked before attending or participating in any events with youth ministry.
